Adoration Home Health, an affiliate of BrightSpring Health Services, seeks a dedicated Admissions Nurse (RN) to join our Fort Wayne team. This role provides patient-centered home health care, assessing needs, creating plans, and coordinating with MD orders.

Ideal candidates have an RN license in Indiana, at least one year of nursing experience, Oasis/home health experience, and a valid driver’s license. You’ll enjoy flexible scheduling, 1:1 patient care, and opportunities for professional growth.

Qualifications

• Registered Nursing Degree (RN) with current state license.
• Minimum 1 year of nursing practice in a patient-care setting.
• Home health or geriatrics experience preferred.
• Valid driver's license and proof of car insurance per policy.
• Current CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Develop home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
