Adoration Health in Fort Wayne, IN, is seeking a Registered Nurse to join our Home Health team as an Admit RN. This full-time role focuses on 1:1 patient care in the home setting with strong emphasis on assessments, care planning, and family education.

Ideal candidates have Oasis experience and a solid Home Health background, with a valid Indiana RN license, CPR certification, and a valid driver’s license. The position offers team support and opportunities for growth within a supportive

Qualifications

• RN license in good standing with state of practice and residence.
• Minimum 1 year nursing practice; home health/geriatrics preferred.
• Valid driver’s license and proof of car insurance.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required
• One year nursing practice in a patient care setting required ; and home health, geriatrics or other related settings preferred
• Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• New nursing graduates may be considered in select markets based on program availability
• Current CPR certification
