Qualifications

• RN license in good standing with state of practice and residence.
• Minimum 1 year nursing practice; home health/geriatrics preferred.
• Valid driver’s license and proof of car insurance.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
