UnitedHealth Group in Gloucester, VA is seeking a Home Health Physical Therapist to provide skilled PT in patients' homes, promoting mobility, safety and independence. You will collaborate with patients, caregivers and a care team to support recovery and prevent hospitalizations.

Requirements include a state PT license, CAPTE-accredited program, and at least 1 year of home health experience or eligibility for mentoring. A valid driver’s license and reliable transportation are required.

Formación

Current license to practice physical therapy in the assigned state.
Graduate from an accredited physical therapy educational program as set by CAPTE.
Minimum 1 year of related PT experience in home health or eligibility for mentoring if less than 1 year.
Driver’s license and access to reliable transportation.

Responsabilidades

Provide skilled PT services at patients' homes to promote mobility and independence.
Perform reexaminations at least every 30 days and adjust interventions as needed.
Collaborate with patients, caregivers and an interdisciplinary care team.
Discharge planning and additional duties as assigned.
