Job summary

Commonwealth Home Health, part of the LHC Group and a member of the Optum family, is hiring a Physical Therapist Assistant to provide treatment, exercise instruction, and patient education under a supervising PT in home care covering Fayette and Jessamine counties. This role requires current licensure/certification to assist in physical therapy, reliable transportation, and weekend availability.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Available to work a weekend rotation
• Current CPR certification or ability to complete within 90 days of hire
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility

Responsibilities

• Follow state-specific laws governing the provision of physical therapy in home care and execute plan of care as defined by supervising PT
• Instruct and aid patients in active and passive exercise, muscle re-education, gait, ADL, transfers, safety, and prosthetic training
• Observe, record, and report patient responses and changes to supervising PT and care team; coordinate care as appropriate
• Educate patient, family, caregiver, and care team in therapy within the scope of physical therapy assistant
• Function effectively in home settings for diverse ages and backgrounds
