Job summary

Responsibilities include following state PT laws, assisting with exercises, recording patient responses, and coordinating with the care team to ensure quality outcomes.

Qualifications

• Current CPR certification or ability to complete within 90 days of hire.
• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.

Responsibilities

• Follow state-specific laws governing home health PT and implement plan of care.
• Instruct and assist patients with active/passive exercises, gait and mobility.
• Observe, document, and report patient responses to treatment to PT and care team.
• Coordinate care with healthcare team and educate patients and caregivers.
