Job summary

Qualifications

• Active LPN licensure in the state of practice.
• Current driver’s license and vehicle insurance, access to reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.
• Home health experience preferred.
• Ability to manage multiple tasks and work independently.
• Strong communication, writing and organizational skills.
• Flexibility to work hours to meet client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care, following infection control procedures.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, communicates with patient, family, physician, and care team.
• Works effectively in diverse home settings regardless of client demographics.
