Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse in Home Health to deliver quality patient care in patients' homes and coordinate with the care team. You will document visits in the EMR and follow the plan of care while adhering to infection control practices.

The role requires current LPN licensure, a valid driver's license, and the ability to manage multiple tasks. CPR is preferred; a comprehensive benefits package is offered.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and access to dependable vehicle or public transportation.
• CPR certification preferred or can be completed within 90 days of hire.
• Home care experience preferred.
• Ability to manage multiple tasks and work independently.
• Good communication, writing and organizational skills.

Responsibilities

• Provide high quality clinical nursing services per plan of care.
• Document patient visits in EMR per regulations and payer requirements.
• Coordinate care with patient, family, physician and care team.
• Work effectively in diverse home settings regardless of client background.
