Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse (LPN) to provide quality in-home patient care within the home health setting. You will follow the plan of care, document visits, and collaborate with the care team to coordinate patient needs.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care.
• Documents patient visits in the EMR per regulatory and payer requirements.
• Participates in care coordination and communicates with patient, family, physician and team.
• Ability to work in diverse home environments and populations.
