Job summary

UnitedHealth Group is seeking an Licensed Practical Nurse (LPN) to provide high-quality clinical care in patients' homes as part of our home health team. The role emphasizes delivering well-coordinated patient care in alignment with the plan of care and regulatory requirements.

The LPN will document visits in the electronic medical record, participate in care coordination, and communicate effectively with patients, families, physicians, and the care team.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Valid driver’s license and vehicle insurance or access to dependable transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record as required by regulatory and payer rules.
• Participates in care coordination, reports patient needs and changes in condition, and communicates with patient, family, physician and care team.
• Able to work in diverse home environments and with patients of varied ages and backgrounds.
