Elk Valley, part of LHC Group and a member of the Optum family, is seeking a Licensed Practical Nurse to provide direct home health nursing as prescribed by the physician and to document and coordinate patient care.

You will work under supervision, document visits, and participate in care coordination to ensure high-quality outcomes. The role offers hourly pay ranging from $22.90 to $34.35, with a comprehensive benefits package and development opportunities.

Qualifications

• Current and unrestricted LPN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or public transport.
• CPR certification preferred or able to obtain within 90 days of hire.

Responsabilités

• Provides high quality clinical nursing services within the scope of practice and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates patient needs and changes in condition.
• Ability to work in various home environments and with diverse patient populations.
