The role supports high-quality care in the patient's home within the assigned geographic area.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance; access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.

Responsabilités

• Documents patient visits thoroughly and completely in the electronic medical record.
• Participates in the agency's care coordination process and communicates updates to the team.
