Job summary

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and access to dependable vehicle or reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.
• Ability to work flexible hours and manage multiple tasks.

Responsibilities

• Provide direct nursing care as prescribed by physician.
• Record clinical notes and progress in EMR with thorough documentation.
• Coordinate other patient care activities/services and communicate with the care team.
• Actively participate in care coordination and report changes in patient condition.
• Follow infection control procedures and ensure quality of care.
