Job summary

HealthPartners is seeking a Resource Home Health Aide to join our Homecare and Hospice team. The role emphasizes compassionate patient care, ADLs support, and family interaction to improve comfort and quality of life.

Responsibilities include assisting with personal care, mobility, medication reminders, and light housekeeping, while adhering to infection control and safety standards. A valid MN driver's license and reliable vehicle are required.

Qualifications

• Certified as a home health aide or nursing assistant with a 75-hour program.
• Six months of successful previous experience as a home health aide or one year as a CNA.
• Able to complete competency testing prior to employment.
• Current MN Driver's License and daily access to a reliable vehicle.
• Computer literacy is required.

Responsibilities

• Assist patients with personal and self-care in the home environment.
• Perform routine observations and treatments as assigned by the nurse.
• Maintain routines for bowel/bladder programs and mobility support.
• Report significant observations to the nurse in a timely manner.
• Demonstrate infection control and safety in patient care.
