Job summary

PurposeCare is seeking compassionate caregivers to provide one-on-one home health and home care support in patients' homes and communities. You will assist with daily living, personal care, mobility, meals, and safety, while reporting to a supervising nurse.

This role offers flexible scheduling (full-time, part-time, PRN), opportunities to grow, and training. Active HHA/CNA preferred or willingness to become certified; reliable transportation required; dementia education included.

Qualifications

• Age 18 or older.
• Active HHA or CNA certification or willingness to obtain certification.
• Willingness to complete competency and skills assessments.
• Ability to follow written care instructions and document services.
• Compassion, patience, and professionalism with vulnerable populations.
• Ability to work independently in home settings.
• Reliable transportation and ability to travel to client homes.
• Ability to lift up to 25 pounds.
• Willingness to complete TB testing, background screening, and onboarding requirements.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education
