Job summary

PurposeCare is seeking dependable, compassionate caregivers to provide one-on-one home health and home care services in patients' homes. You will assist with personal care, daily living activities, and light household tasks while supporting families and supervising nurses.

Qualifications

Active HHA or CNA certification or willingness to obtain certification.
Willingness to complete competency and skills assessments.
Ability to follow written care instructions and document services.

Responsibilities

Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting.
Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan.
Prepare meals and assist with feeding as needed.
Support medication self-administration through reminders (no independent med administration).
Monitor and report patient condition and changes to the supervising nurse.
Take and record basic vital signs when trained and assigned.
Maintain a clean, safe, and orderly client environment.
Assist with linens and light client laundry as appropriate.
Travel reliably to patient homes and assigned visits across your service area.
Document care provided according to company and regulatory standards.
