Job summary

PurposeCare is seeking compassionate in-home care professionals to provide one-on-one support in patients' homes across the Midwest. You will assist with daily living, mobility, and personal care, delivering safe, respectful, and personalized care on every visit.

Active HHA or CNA certification is preferred, or willingness to obtain it. Flexible shifts, dependable scheduling, and opportunities to grow in home health care are offered.

Qualifications

• Active HHA or CNA certification or willingness to obtain certification.
• Ability to travel to client homes and lift up to 25 pounds.
• Comfort with in-home, patient-facing care and documentation.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education
