Job summary

PurposeCare is seeking dependable Home Health Aides/CNAs to provide compassionate, one-on-one personal care in patients' homes. You will assist with daily living, mobility, meals, and safety, while documenting care and communicating with supervising nurses.

Ideal candidates are licensed or willing to become licensed, reliable, patient, and able to travel to client homes across the service area. Flexible schedules, ongoing training, and opportunities to grow within purpose-driven home care.

Qualifications

• Must have active HHA or CNA certification or willingness to obtain certification.
• Compassion, patience, and professionalism with vulnerable populations.
• Ability to work independently in home settings and follow care plans.
• Strong communication with patients, families, and nurses.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting.
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders (no independent med administration).
• Monitor and report patient condition to supervising nurse.
• Record basic vital signs when trained and assigned.
• Maintain a clean, safe, and orderly client environment.
• Assist with linens and light client laundry as appropriate.
• Use safety equipment properly (rails, supports, stools, etc.).
• Document care provided according to company and regulatory standards.
• Travel reliably to patient homes and assigned visits across service area.
• Complete required in-service training, including annual dementia education.
