Qualifications

• Must hold state and federal Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read care plans and document services.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to work independently with strong time management.
• Empathy, patience, and professionalism.

Responsibilities

• Assist patients with daily living activities (bathing, dressing, grooming).
• Support mobility and transfers per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding.
• Take and record vital signs when required.
• Assist with medication self-administration as directed.
• Document care in the EMR accurately.
• Participate in team trainings and care coordination.
