Job summary

VitalCaring is hiring a Home Health Aide (HHA) to provide compassionate, hands-on care in patients' homes. You will assist with daily living activities, mobility, safety, and documentation in the EMR system, collaborating with nurses and care teams to ensure high-quality visits.

Ideal candidates are dependable, empathetic, with strong time-management skills, and able to work independently in a home-based setting, while meeting state requirements and maintaining CPR certification.

Qualifications

• Meets state and federal Home Health Aide requirements.
• At least 18 years of age.
• Ability to read care plans and document services.
• Current CPR certification.

Responsibilities

• Provide compassionate, hands-on care that supports patient comfort, safety, and independence.
• Assist with daily living activities (bathing, dressing, grooming, toileting).
• Support mobility, transfers, and basic exercises per care plan.
• Help maintain a clean, safe home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when required and trained to do so.
• Assist with medication self-administration as directed.
• Observe and report changes to the clinical team.
• Document care provided accurately and timely in the EMR system.
• Participate in team meetings, trainings, and care coordination activities.
• Provide compassionate support to patients and families during visits.
