VitalCaring seeks a compassionate Home Health Aide to provide personalized in‑home care, ensuring patient comfort, safety and independence. You will follow care plans, document services, and support daily activities in patients’ homes.

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Ability to work independently with strong time management skills.
• Demonstrates empathy, patience, and professionalism in all interactions.

Responsibilities

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting.
• Support mobility, transfers, and basic exercises in alignment with the care plan.
• Help maintain a clean, safe, and comfortable home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when required and trained to do so.
• Assist with medication self‑administration as directed.
• Observe and report changes in patient condition to the clinical team.
• Document care provided accurately and timely within the EMR system.
• Participate in team meetings, trainings, and care coordination activities.
• Provide compassionate support to patients and families during each visit.
