VitalCaring is seeking a compassionate Home Health Aide to provide hands-on support, comfort, and dignity to patients in their homes. You will work independently to deliver high-quality, personal care and build meaningful connections with patients and families.

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Ability to work independently with strong time management.

Responsibilities

• Assist patients with daily living activities (bathing, dressing, grooming, toileting).
• Support mobility and transfers per care plan.
• Help maintain a clean, safe home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when required and trained to do so.
• Assist with medication self-administration as directed.
• Observe and report changes in patient condition to the clinical team.
• Document care provided accurately and timely within the EMR system.
• Participate in team meetings, trainings, and care coordination activities.
• Provide compassionate support to patients and families during each visit.
