VitalCaring is seeking a dedicated Home Health Aide to provide compassionate, hands-on care for patients in their homes. You will support daily living activities, assist with mobility and safety, document care, and collaborate with nurses and care teams to ensure high-quality, patient-centered service.

Qualifikationen

• Meets state Home Health Aide certification requirements.
• At least 18 years old.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Aufgaben

• Provide compassionate hands-on care that supports patient comfort, safety, and independence.
• Assist patients with daily living activities (bathing, dressing, grooming, toileting).
• Support mobility, transfers, and light exercises per care plan.
• Maintain a clean, safe home environment for patients.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when trained to do so.
• Assist with medication self-administration as directed.
• Observe and report changes to the clinical team.
• Document care provided accurately in the EMR.
• Participate in care coordination meetings and trainings.
