Job summary

VitalCaring Group is seeking a compassionate Home Health Aide to provide hands-on care in patients' homes. You'll support daily living activities, monitor safety, and help maintain a clean, comfortable home.

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification.
• Valid driver''s license, reliable transportation, and auto insurance.
• Ability to work independently with strong time management skills.
• Demonstrates empathy, patience, and professionalism in all interactions.

Responsibilities

• Provide hands-on care that supports patient comfort, safety, and independence.
• Assist with daily living activities: bathing, dressing, grooming, toileting.
• Support mobility, transfers, and basic exercises per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when required and trained to do so.
• Assist with medication self-administration as directed.
• Document care provided accurately and timely in the EMR system.
• Participate in team meetings, trainings, and care coordination activities.
• Provide compassionate support to patients and families during each visit.
