Job summary

The Lakes Home Care is seeking a compassionate Home Health Aide to assist patients in their homes per a care plan. Responsibilities include helping with daily living tasks, monitoring vitals, safe transfers, and documenting observations for the supervising nurse.

Requirements include CPR certification and core trainings; you will follow the plan of care, maintain safety, and communicate effectively with families and the office. Part-time position with flexible hours and competitive pay.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains CPR certification as required.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist with baths, oral hygiene, dressing, toileting and feeding according to the plan of care.
• Take and record vital signs as ordered (temperature, pulse, respiration, BP).
• Safe transfer, ambulation, and ROM as directed in the plan of care.
• Exercise according to the plan of care and assist with simple procedures if competencies exist.
• Handle patient laundry and maintain safety using equipment properly.
• Observe and report changes; follow up with nurse supervisor.
• Communicate clearly with client, family/caregiver and office.
• Meet safety needs of patients and provide home safety education.
