Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes, following an established care plan. The role includes assisting with hygiene, mobility, and activities of daily living.

Responsibilities include observing patient condition, recording vitals, and communicating with the nurse supervisor. The position offers flexible hours and a supportive team environment dedicated to patient comfort and safety.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with and per plan of care, including baths, hygiene, dressing, toileting, feeding.
• Take and record vital signs as ordered (temperature, pulse, respiration, BP).
• Assist with safe transfer and ambulation; support range of motion and simple procedures as ordered.
• Perform patient laundry and observe safety practices; use equipment safely.
• Observe and document patient condition; report changes to nurse supervisor.
• Communicate clearly with client, family/caregiver, and office.
