Job summary

Assured Home Health & Hospice, part of the Optum family, seeks a Home and Community Based Services Aide to provide personal hygiene assistance and support daily living activities for an individual at home. You will help with bathing, grooming, dressing, ambulation, meal prep, and accompany appointments as needed.

Responsibilities include monitoring client condition, recording vital signs, maintaining confidentiality, operating medical equipment safely, and responding promptly to client needs.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation.
• Current CPR certification.
• Ability to work flexible hours and independently.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
