Job summary

LifeCare Home Health and Hospice Family in Florida is seeking compassionate Home Health Aides to support patients in their own homes. You will monitor health, assist with daily living activities, and collaborate with the care team to uphold our mission and quality standards.

Qualifications

• High School graduate or equivalent.
• Home Health Aide Certificate or other similar certification in good standing from an accredited training course that meets the standards established by Health and Human Services
• Minimum of one year (1+) of experience as a home health aide or CNA
• Strong English language both written and verbal
• Active CPR certification required
• Valid driver’s license, reliable transportation and insurance required

Responsibilities

• Monitor and record patient condition; assist with plan of care.
• Provide personal services and help with ADLs.
• Assist with housekeeping, meals, and shopping.
• Support families by teaching lifting/turning techniques.
• Communicate with Care Team to ensure continuity of care.
• Enter patient data into EMR and notify leadership of changes.
• Perform on-call duties as assigned.
• Maintain a safe, clean home care environment.
• Adhere to home care policies and professional standards.
• Complete required CEUs.
