Job summary

CenterWell Home Health in Florida is seeking a compassionate Home Health Aide to provide in-home patient care under RN supervision. You will assist with activities of daily living, monitor vital signs, support mobility, and document care given to ensure continuous, high-quality home health services.

Requirements include a CNA or HHA certificate within the last 24 months, state certification, a valid driver’s license and reliable transportation, plus at least one year of prior HHA/CNA experience

Qualifications

• Must complete CNA or HHA program within the last 24 months.
• Must meet applicable state certification requirements.
• Valid driver’s license, auto insurance, and reliable transportation required.
• Minimum one year experience as HHA or CNA in hospital, nursing home, home health/hospice.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with self-administered medications and monitor dosing.
• Take and report vital signs and observe changes.
• Support hygiene, grooming, and daily living activities.
• Assist feeding and document intake and nutrition.
• Support mobility, transfers, positioning and exercises per care plan.
• Document care interactions with patients and caregivers.
• Follow infection control practices and maintain a safe home environment.
• Perform light housekeeping tasks as needed and allowed by MD orders.
