Job summary

Herewith is seeking compassionate In-Home Dialysis Care Partners to support patients receiving dialysis at home. You will ensure a safe environment, assist with treatment prep, and monitor overall wellbeing, matched to patients by skill fit and personality with clinical team support.

Role offers consistent weekly hours at $25 per hour, plus training and placement assistance. You will work independently while remaining integrated with a clinical care team to ensure quality patient care.

Qualifications

• Dialysis care experience in home or clinical settings.
• Familiarity with home dialysis machines preferred.
• Empathetic, reliable, and detail-oriented.
• Strong interpersonal communication and collaboration with clinical teams.
• Must pass background check and vetting via Herewith platform.

Responsibilities

• Assist with patient setup for dialysis treatments.
• Provide companionship and ensure schedule adherence.
• Monitor safety and assist with light ADLs.
• Maintain communication with nurses and care coordinators.
• Document concerns observed during support hours.
