Job summary

Herewith seeks a Dialysis Care Partner to support a 69-year-old patient at home, following dialysis training and assisting with home hemodialysis. You’ll provide daily monitoring and one-on-one care, ensuring safety and comfort during treatments while building a trusted care routine with the client and family.

Training is provided by a clinical team, and your role emphasizes dignity, reliability, and compassionate presence in a home setting, with consistent hours and limited agency rotation.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (Tableu or NxStage is a plus).
• Open to in-home dialysis support and willing to complete training.
• Steady, supportive and patient, especially during adjustment.

Responsibilities

• Assist with dialysis treatment setup (Tableu or NxStage dialysis machine).
• Keep the treatment environment clean, organized, and safe.
• Offer companionship and emotional reassurance.
• Help the patient remain on schedule with treatment times.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
