Community Assessment Nurse (RN)

Location: North Central Massachusetts

Schedule: Full-Time (35 Hours Per Week)

Work Environment: Remote Community-Based Position

Compensation: $34-$37 per hour

What You'll Do

As a Community Assessment Nurse, every day is different. You may:

• Visit older adults in their homes to assess their health, safety, and support needs.
• Build meaningful relationships with consumers and family caregivers.
• Complete comprehensive nursing assessments and identify areas of concern.
• Recommend services and supports that help consumers remain safely in their homes.
• Collaborate with care managers, physicians, hospitals, rehabilitation facilities, and community providers.
• Educate consumers and caregivers about community resources and service options.
• Assist with transitions from hospitals, rehabilitation facilities, and nursing facilities back into the community.
• Advocate for consumers and help them navigate healthcare and long-term care systems.
• Monitor changing needs and recommend adjustments to care plans when appropriate.
• Maintain accurate documentation and complete required assessments in accordance with program guidelines.

Required Qualifications
