Job summary

Guardian Angel Senior Services seeks an experienced Registered Nurse (RN) to join our home care team serving Boston and the Greater Boston area. This part-time or per diem position offers flexible scheduling, block scheduling options, mileage reimbursement, and strong office support for an independent field-based role with one-on-one client visits.

The RN will perform in-home assessments, develop care plans, educate and mentor caregivers, and ensure timely documentation while delivering

Qualifications

• Massachusetts RN license is required.
• Experience in home care, community health, case management, or visiting nurse roles is preferred.
• Strong assessment and clinical judgment skills are essential.

Flexible Scheduling Options

Block Schedule

• Choose the days and hours you're available
• Paid for your scheduled block of time
• Our office coordinates client visits, travel time, and documentation

Guaranteed Hours

• Guaranteed 5–8-hour workdays available
• Collaborative scheduling with office staff
• Typical expectation of approximately four client visits during a five-hour shift, depending on travel distance

What You'll Do

As a Home Care RN, you will:

• Complete comprehensive in-home client assessments
• Develop, review, and update individualized care plans
• Conduct caregiver orientation and quarterly supervisory visits
• Educate, coach, and support caregivers to ensure quality care
• Complete documentation within required timeframes
• Monitor client progress and communicate changes to the care team
• Collaborate with office staff to ensure continuity of care
• Participate in caregiver education and in-service training
• Perform routine nursing visits as needed, including medication management, blood pressure checks, catheter changes, and other nursing services within your scope of practice
• Provide hands-on assistance during urgent situations when needed

Aging Services Access Point (ASAP) Responsibilities

For clients receiving services through Aging Services Access Point (ASAP) programs, you will:

• Complete in-home assessments and develop Home Health Aide Plans of Care
• Match caregiver skills with client care needs
• Orient and supervise caregivers assigned to ASAP clients
• Complete quarterly supervisory visits and annual reassessments
• Collaborate with ASAP nursing staff regarding client status and care plan updates
• Identify changes in client condition and advocate for appropriate services
• Report required incidents and changes according to ASAP guidelines

Qualifications

• Current Massachusetts Registered Nurse (RN) license
• Home care, community health, case management, or visiting nurse experience preferred
• Strong assessment and clinical judgment skills
• Excellent communication and documentation skills
• Ability to work independently
• Reliable transportation, valid driver's license, and automobile insurance
