Job summary

Richmond Community Services is expanding its care model and seeks a Community Nurse (RN) to connect, coordinate, and impact the lives of the people we support in a meaningful way.

This role offers a Monday–Friday schedule with no weekends, enabling consistent, person-centered care and collaboration with our Medical Services Team. Comprehensive benefits, generous vacation, and a 403(b) retirement plan accompany the position.

Qualifications

• Active, valid NYS RN license.
• Minimum 2 years of nursing experience.
• OPWDD experience is a major plus.

• Conduct health assessments and ongoing monitoring for the people we support.
• Coordinate and schedule medical appointments and follow-ups.
• Partner with our Medical Services Team to ensure seamless, high-quality care.
• Collaborate with interdisciplinary teams to support person-centered outcomes.
• Advocate for and promote the overall health, wellness, and independence of the people we support.
