Job summary

Heartlite Hospice, part of the Optum family, is seeking a Home and Community Based Services Aide to support clients at home. You will provide personal care, assist with daily activities, and help with household tasks while documenting health information and ensuring safety in the client’s home.

Qualifications

• Must have current driver's license, vehicle insurance, and reliable transportation.
• CPR certification preferred or able to complete within 90 days of hire.
• 6+ months of home care experience preferred.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to work in diverse home situations regardless of age, race, creed, color, sex, disability, or financial condition of the client.
