Job summary

Halcyon Home is seeking a dedicated and compassionate Home Health Field Physical Therapist PRN to support our geriatric home health population in Northern Dallas/Richardson/Plano/Frisco, TX. Competitive pay and a supportive team await.

Current PT license required; home health experience preferred. You will perform assessments, develop and adjust plans of care, supervise therapy staff, and educate patients and families while working independently in a fast-paced schedule.

Qualifications

• Current PT license required.
• Experience in home health or related healthcare setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Performs ongoing clinical assessment, including home health assessments, to identify patient needs and level of functioning.
• Develops and evaluates the plan of care in partnership with the patient and caregiver(s).
• Documents clinical notes and completes charting in a timely manner.
• Provides therapy services ordered by the physician as part of the plan of care; supervises team members as appropriate.
• Implements infection control practices and patient safety measures.
• Assists and instructs patients and families in transfer techniques and equipment use.
