Job summary

Halcyon Home is seeking a dedicated Home Health Physical Therapist PRN to support our geriatric population in Waco, TX and surrounding areas. You will assess, plan, and coordinate in-home therapy to improve function and comfort while delivering compassionate care.

As a Registered PT, you will collaborate with patients, families, and a caregiving team, providing education and hands-on treatment. Join a supportive, patient-centered organization that values dignity and teamwork.

Qualifications

• Current PT license required.
• Home health experience preferred.
• Strong clinical and assessment skills required.
• Excellent communication and interpersonal abilities required.
• Ability to work independently and manage time effectively.

Responsibilities

• Performs ongoing clinical assessment, including OASIS evaluations, to identify patient needs and determine level of functioning.
• Develops and evaluates the plan of care in partnership with the patient, representative, and caregiver(s).
• Documents, prepares, and completes clinical notes and orders in a timely manner; revises the POC based on evaluation data.
• Provides services that are ordered by the physician and oversees other team members in patient care.
• Applies infection control and standard precautions in coordinating patient care activities.
• Provides supervision and guidance to PT assistants and home health aides as appropriate.
