Qualifications

• Current PT license is required.
• Experience with Homecare Homebase EMR is a plus.
• Home health or related healthcare experience preferred.
• Strong clinical and assessment skills are essential.
• Excellent communication and interpersonal abilities are required.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform ongoing clinical assessments, including OASIS, to identify patient needs and plan care.
• Develop and evaluate the plan of care with patient and caregivers.
• Document clinical notes, orders, and updates promptly.
• Provide services ordered by physicians, supervising team members as needed.
• Apply infection control and safety practices in patient care.
• Supervise PT aides and home health aides as appropriate.
• Plan and deliver treatment for neuromuscular, neurological, and orthopedic issues.
• Communicate with physicians and other health professionals about current plan of care.
• Meet continuing education requirements for licensure.
