Halcyon Home is seeking a dedicated and compassionate Home Health Field Physical Therapist PRN to support our growing geriatric home health population in Mesquite/Garland/Eastern Dallas, TX and the surrounding area. Competitive pay and wonderful team support.

Qualifications

• Current PT license required.
• Home health experience preferred.
• Excellent communication and interpersonal abilities.

Responsibilities

• Performs ongoing clinical assessment including OASIS to identify patient needs and level of functioning.
• Develops and evaluates the plan of care with patient and caregiver(s).
• Documents and completes clinical notes, phone orders, and other records in a timely manner.
• Provides services ordered by the physician as indicated in the plan of care; oversees team members as appropriate.
• Applies infection control and standard precautions during patient care activities.
• Accepts assignments consistent with education and competence.
• Provides supervision to PT assistants and home health aides as appropriate.
• Plans and delivers treatment for neuromuscular, neurological, and orthopedic disorders.
• Communicates with physicians and other health care practitioners regarding the plan of care.
• Meets mandatory continuing education requirements of the agency and licensing board.
• Assists families in transfer techniques and use of equipment; provides education.
