Job summary

Halcyon Home is seeking a dedicated Home Health Field Physical Therapist PRN to support our geriatric population across Northern Dallas/Richardson/Plano/Frisco, TX. You will perform patient assessments, develop plans of care, and coordinate with physicians and caregivers to ensure high-quality, compassionate care.

Ideal candidates have home health experience, strong clinical skills, and the ability to work independently in a fast-paced environment.

Qualifications

• Current PT license
• Experience in home health or related healthcare setting preferred
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively

Responsibilities

• Performs ongoing clinical assessment including OASIS at appropriate time points
• Develops and evaluates the plan of care with patient and caregiver(s)
• Documents and updates clinical notes and orders timely
• Provides services ordered by physician and oversees team members as applicable
• Instructs patients and families in transfer techniques and equipment use
• Supervises PT assistants and home health aides as appropriate
• Plans and provides treatment for neuromuscular, neurological and orthopedic disorders
• Communicates with physicians and other health practitioners about the plan of care
• Meets continuing education requirements
• Ensures safety and infection control practices
