Halcyon Home is seeking a dedicated and compassionate Home Health Field Physical Therapist PRN to support our growing geriatric home health population in Tarrant County/HEB and the surrounding area. We offer competitive pay and wonderful team support.

Qualifications

Current PT license in good standing.
Experience in home health care preferred.
Strong clinical and assessment skills.
Excellent communication and interpersonal abilities.
Ability to work independently and manage time effectively.

Responsibilities

Performs ongoing clinical assessment, including relevant measures at appropriate time points.
Develops and evaluates plan of care in partnership with patient and caregiver(s).
Documents and completes clinical notes and orders in a timely manner.
Provides services ordered by physician as indicated in the plan of care.
Oversees other team members when applicable to meet patient needs.
Applies infection control and standard precautions in patient care.
Assists and instructs patients and families in transfer techniques and use of equipment.
Provides supervision to PT assistants and home health aides as appropriate.
Communicates with physicians and other health care practitioners about the plan of care.
Meets continuing education requirements of the Agency and licensing board.
