Job summary

OneWell Health Care seeks a dedicated Caregiver/DSP in Plymouth, PA. This field-based role supports individuals with IDD in home and community settings, helping with daily living, skill development, and community participation.

You will implement ISP/IPP plans, monitor health and safety, and maintain accurate documentation while coordinating with the interdisciplinary team. Flexible scheduling and local travel are required, with strong confidentiality and regulatory compliance a must.

Qualifications

• High school diploma or equivalent.
• Preferred: caregiving, home health, or direct support experience with IDD/IDD.
• English proficiency required; bilingual skills a plus depending on program needs.

Responsibilities

• Provide hands-on, person-centered support per care plans.
• Assist with daily living activities, meal prep, mobility, and medications as authorized.
• Support transportation to appointments and community activities.
• Maintain documentation and participate in trainings/meetings.
• Follow HIPAA and program policies; ensure health, safety, and dignity.
