Job summary

WellLife Network is seeking a Direct Support Professional to provide hands-on care and advocacy for individuals with developmental disabilities in New York. You will assist with daily living, health and wellness, mobility, and community activities while documenting progress and collaborating with a care team.

The role emphasizes compassionate teamwork, accurate record-keeping, and ongoing opportunities for professional development within a supportive nonprofit environment.

Qualifications

• High School diploma or equivalent required.
• Experience with adults with intellectual/developmental or other behavioral health disabilities preferred.
• Ability to provide direct personal care and daily living support.
• Willingness to work as part of a supportive team.

Responsibilities

• Personal Care & Daily Living Support: Assist with bathing, dressing, grooming, and toileting; help with meals and household tasks; maintain a safe home environment.
• Health & Wellness: Administer medications as directed; support medical appointment attendance and treatment plans.
• Mobility & Accessibility: Assist with walking, transfers, and use of mobility aids; ensure safe navigation.
• Behavioral & Emotional Support: Implement respectful strategies to support behaviors, provide encouragement and reassurance.
• Community & Social Engagement: Plan and participate in recreational, educational, and social activities; foster community involvement.
• Communication & Advocacy: Assist communication, help individuals express needs, advocate for rights and services.
• Collaboration & Documentation: Work with healthcare providers and team; maintain daily notes, progress reports, incident reports, and other documentation.
