Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. The role involves implementing care plans, promoting independence, and ensuring health and safety.

The position requires a high school diploma or equivalent, with preferred prior caregiving experience. Roles include flexible scheduling, daily living assistance, and documentation duties in a field-based home health environment.

Qualifications

• High school diploma or equivalent is required.
• Experience in caregiving or home health is preferred.
• Ability to follow written care plans and documentation guidelines.

Responsibilities

• Provide consistent, respectful, and high-quality support per each individual's care plan.
• Assist with personal care and daily living activities, meal preparation, nutrition, mobility, and medication assistance as authorized.
• Monitor health, safety and emotional well-being; support community participation and transportation as needed.
• Maintain accurate and timely documentation and participate in trainings and team meetings.
