Job summary

Creative Foundations in the community is seeking Direct Support Professionals to provide one-on-one support to individuals with developmental disabilities, both at home and in their communities. This role involves helping with daily living skills, enabling participation in social activities, and accurately documenting care and progress, while upholding safety, dignity, and independence.

Pay includes $18/hour during waking hours and $11/hour for sleep shifts, with a $1,000 sign-on bonus offered

Qualifications

• High school diploma or equivalent.
• Successful background check and required training.
• At least 21 years old.
• Med Cert 1.
• Valid driver’s license with reliable transportation.
• Ability to work safely in clients’ homes and communities.
• Compassion, reliability and a desire to help others.

Responsibilities

• Provide one-on-one support to individuals in homes and communities.
• Assist with daily living activities such as cooking and personal care.
• Encourage participation in social and community activities.
• Document care, activities and progress accurately.
• Communicate with the care team to ensure consistent support.
