Job summary

Harvest Crossing Post-Acute in California is seeking a Certified Occupational Therapy Assistant (COTA) to join our skilled nursing team. You will implement treatment plans under the supervision of an OT, assist residents with ADLs and therapeutic activities, and document progress to support licensing and care goals.

We welcome new graduates and experienced COTAs who are passionate about rehab, teamwork, and patient-centered care.

Qualifications

• Must have or be eligible for a California COTA license.
• CPR certification is required.
• Experience in SNF or rehab settings is preferred.
• Graduating students are encouraged to apply.

Responsibilities

• Implement established OT treatment plans under supervision of a licensed OT.
• Assist residents with ADLs, mobility, and therapeutic activities.
• Monitor patient responses and document progress.
• Communicate with residents, families, and interdisciplinary rehab team.
• Maintain accurate documentation for licensing, reimbursement, and compliance.
• Participate in interdisciplinary team meetings and care planning.
• Support safety, HIPAA, and professional standards.
