Job summary

PACE SEMI is seeking a Community Living Associate to provide supportive services to residents in the community setting, during transport, and at the day health center. You will follow each participant's care plan and work under supervision of the Community Living Manager and Lead CLA.

Responsibilities include assisting with personal care, meals, activities, and coordination with the nursing team. Must have a valid Michigan driver’s license and CPR certification, with wage considerations based on

Qualifications

• H.S. diploma or GED required.
• One year experience with a frail or elderly population.
• Current CPR card required.

Responsibilities

• Provide supportive services to participants in community setting, transport, appointments, and day health center.
• Assist with activities, meals, toileting, and daily living tasks per care plan.
• Coordinate care with the DHC and clinical team and document progress.
• Respond to emergencies per policies and maintain care plan compliance.
• Maintain records and report deviations in schedule or care needs to supervisor.
