Job summary

PACE is seeking a Community Living Associate (CLA) to support residents in the community living setting, transport, appointments, and day health center under supervision. Responsibilities align with each participant’s care plan and CMS-approved competencies.

The CLA provides personal care, assistance with meals, toileting, homemaking, and coordination with the DHC and nursing staff, ensuring safety and respectful care in a CMS-compliant program in Michigan.

Qualifications

• H.S. diploma or GED required; CNA certification preferred.
• Must have one year experience with frail/elderly populations.
• Completion of a formal nursing assistant training program required.
• Ability to establish interpersonal relationships and work with diverse groups.

Responsibilities

• Provide personal care up to 2-person assist per plan of care.
• 1:1 interaction in day health center and community living as outlined.
• Assist with activities, meals, toileting, and mobility per plan.
• Coordinate care with participant, DHC, and supervisor; document accurately.
• Report deviations, maintain schedules, and respond to emergencies per policies.
