Job summary

The Community Living Associate (CLA) at PACE SEMI provides supportive services to residents in a community setting, including during transport and appointments, under supervision of the manager, RN, or Lead CLA. Duties align with each participant's care plan and emphasize comfort, independence, and respectful care.

Responsibilities include personal care up to 2-person assist, 1:1 interaction, meal and activity assistance, and documentation.

Qualifications

• H.S. diploma or GED required.
• Current CPR certification preferred.
• Valid Michigan driver's license required.
• One year of experience with frail or elderly population preferred.
• Experience in long-term care, hospital, home care or PACE setting is desirable.

Responsibilities

• Provide personal care up to 2-person assist.
• Offer 1:1 interaction in DHC and CL as outlined in the plan of care.
• Assist with activities, meals, toileting, and mobility.
• Coordinate care with participants and DHC per care plan.
• Maintain accurate timely documentation of services provided.
• Report deviations from schedule and any emergencies to supervisor.
