Job summary

PACE Southeast Michigan is seeking a Community Living Associate to provide compassionate support to residents in a community setting, including assistance with daily activities, meals, and coordination of care under supervision. The role emphasizes safety, documentation, and adherence to CMS guidelines.

The position requires a high school diploma or GED, a Michigan driver’s license, and CPR/BLS training, with nursing assistant training and relevant experience preferred.

Qualifications

• HS diploma or GED required.
• Active Michigan driver's license.
• Current CPR card required.
• One year experience with frail/elderly population preferred.
• Experience in long-term care, hospital, home care, or PACE setting.
• Completion of basic nursing assistant training program.
• Certificate of completion from a formal training program (Direct Care, Nurse Assistant, etc.).
• Mandatory in-service training (infection control, TB, safety, BLS).
• Ability to establish/interdepartmental relationships.
• Respect for participants/co-workers' well-being.

Responsibilities

• Provide care in community setting, transport, and appointments as per care plan.
• Assist with activities of daily living and meal preparation.
• Assist with toileting and incontinence care.
• Coordinate care with DHC based on care plan.
• Maintain documentation of services provided.
• Attend training and adhere to CMS competencies.
• Respond to emergencies per policies.
• Perform duties with flexibility and respect for diverse backgrounds.
• Carry out homemaking and other duties as assigned.
• Rounds every 2 hours after 7pm–7am.
